Forms and registration
- Online patient registration — patients can complete this before their first visit so their details and history are with us ahead of time.
- Nasal airway laboratory (NOSE Lab) referral — PDF for objective nasal airflow and allergy testing.
What to send
- A referral letter addressed to the specialist (or to the clinic if unsure).
- Relevant imaging reports and results — a non-contrast CT sinus for chronic sinus problems, audiology for hearing, ultrasound for a neck lump — are helpful but not essential.
- Tertiary and complex problems can be directed to our Darlinghurst clinic.
